POLICIES

The following information outlines the policies and guidelines in place at the Fallingbrook Dental Centre.

New Patients
Fallingbrook Dental Centre takes great pride in providing our patients with the very best dental care; new patients are welcome by appointment only. At your new patient visit our dental team will complete: a full oral exam, perform x-rays as needed, conduct cancer and gum disease screening, do a thorough scaling (cleaning), as well as polishing, fluoride and iodine treatment if required.

Our office is perfume free due to staff allergies.

Billing Policy
Our dental insurance claims are filed electronically to ensure prompt payment to you from your insurance company. We kindly ask for full payment at the end of each visit. Our methods of payment include: Visa, M/C, Cash or Debit.
There will be an administration charge of $25 for any returned cheque.

Failed Appointment Guidelines
Our practice is dedicated to your quality care and is pleased to reserve this appointment time for you. In order to meet the scheduling requests of all of our patients, we have established guidelines regarding appointment scheduling. Many patients need our services and missed appointments affect everyone. So that we may provide care to all of our patients, we require a minimum of 24 hours notice for appointment changes. A charge will be applied for same day cancellation or missed appointments without prior notification. Our protocol for late appointments will be decided on an individual basis.

If you are a current office patient and you have a dental emergency on the weekend that cannot wait, we have an emergency pager service. Just call our office number and you will be given instructions.

Warranty
Our office policy regarding replacement of broken or lost restorations is no charge for replacement if the filling was done in the last year.

Our office policy regarding replacement of crowns (caps), bridges and veneers is 5 years against loss or fracture of the prosthesis. This does not cover recurrent caries (cavities).

Privacy of your personal information is an important part of our office providing you with quality dental care. We understand the importance of protecting your personal information. We are committed to collecting, using and disclosing your personal information responsibly. We also try to be as open and transparent as possible about the way we handle your personal information. It is important to us to provide this service to our patients.
